A Coordinated Approach to Hospital Care
We hope you never need to go to the hospital. But we realize, life happens…and sometimes medical conditions become severe enough to require hospitalization.
AdvantageCare Physicians has a special Hospitalist program in more than 20 hospitals throughout Manhattan, Brooklyn, Queens, Staten Island and Long Island. This program is designed to provide coordinated care for our patients who need to be hospitalized.
What is a Hospitalist?
A Hospitalist is a medical doctor who specializes in caring for patients in the hospital. They provide skilled medical care to patients 24 hours a day, seven days a week. Our Hospitalists partner with and serve as an extension of your Care Team while you are in the hospital. If you are admitted to an AdvantageCare contracted hospital, our Hospitalists will:
- Notify your Care Team that you are in the hospital and let them know why you were admitted
- Update your Care Team on any changes in your health
- Coordinate your specialist care
- Continuously monitor your progress and let you know your test results
- Keep your family informed of your status, if you wish

Our Care Team
Our relationship with you begins by assigning you to a Care Team that includes your primary care physician, a nurse, medical assistant and a patient service representative.
