This notice describes the health information privacy practices of our medical group, its medical staff and affiliated health care providers who jointly perform health care services with our medical group, including physicians and physician groups who provide services at our facilities. A copy of our current notice will always be posted at all registration and/or admission points. You may also print this notice, request a copy from any staff member or call the administrative department of the medical office where you receive your care.
