How do I schedule my doctor appointment?
For regular and follow-up appointments with primary care physicians and select specialists, click the “SCHEDULE AN APPOINTMENT” button in the upper right corner of this page and get access to doctors at any of our 36 medical offices. What’s the fastest way to refill my prescription?
If you need to refill a prescription, the fastest way is to contact your pharmacy directly. Your pharmacist can also answer any questions you have about your medication.

You can also request a prescription refill by signing in to your myACPNY patient portal and submitting your request online. Not signed up for myACPNY yet? Register here.

What’s the best way to contact my Care Team?
The fastest, easiest way to contact your Care Team is by sending an email through your secure myACPNY patient portal. That way, you don’t need to worry about sticking close to the phone for a return call if your Care Team is not available to speak with you right away. Not signed up for myACPNY yet? Register here.

How do I get a referral to a specialist?

What forms of insurance do you accept?
ACPNY works with most health insurance plans. More information can be found here.  We also offer patients the ability to self-pay. ACPNY accepts cash, debit and credit cards as payment for your visit. 

How can I get an answer to a billing question?

How can I get directions to one of your medical offices?
