Everything you need to know

Matters relating to HIPAA are collaboratively managed by the Compliance and Information Technology departments. These departments implement policies and procedures to:

  • Manage the acquisition, use, and disclosure of protected health information (PHI)
  • Investigate privacy and security complaints and breaches
  • Ensure individuals receive treatment and services in accordance with the Notice of Non-discrimination policy.
  • Provide regulatory guidance to ACPNY staff to ensure that it complies with federal, state, and local laws and regulations, as well as its own Compliance policies.

What is HIPAA?

The Health Insurance Portability and Accountability Act (HIPAA) was signed to keep patients’ medical information safe. The goals of HIPAA are to protect health insurance coverage for workers and their families when they change or lose their jobs, and to protect health data integrity, confidentiality, and availability.

Who can access my health information?

HIPAA’s “Privacy Rule” gives you the right to access your health information and helps ensure that your protected health information cannot be viewed by others. While ACPNY may use or share your information for the purposes of payment, treatment, and health care operations or as otherwise allowed or required by law, we will otherwise not share your information unless we have permission.

Can I authorize others to access my health information?

You can designate family members or other persons involved your care as your HIPAA Representative. This allows them to speak with providers about your care and gives them permission to access your health information. To designate someone as your HIPAA Representative, you must complete, sign, and submit the HIPAA Representative Form.

To authorize access to or release a copy of your medical records to a family member or another person or entity, you must complete the Authorization to Use and Disclose Protected Health Information form.

Please Note: A per-record service charge of $6.50 may apply when requesting paper copies of your chart.

Request medical records online—securely and conveniently

ACPNY has partnered with Verisma, a service that allows you to complete, e-sign, and submit your requests electronically from the comfort of your home (or anywhere else) 24/7. With Verisma, you avoid any need for hardcopy forms, faxing or mailing. To request your own medical records from Verisma, start here. If you are requesting records on behalf of an ACPNY patient, start here.

The fastest, most convenient way to locate your medical record is to access myACPNY, our free online patient portal. Simply sign in with your username and password and click on the main menu on the top of the screen to search for your medical records.

If you do not have a sign-in or activation code, click on "Sign Up Online" and follow the onscreen instructions.

To make a request by mail or fax please access our Authorization Form, giving our medical staff appropriate permission to release your chart. Please print out the form, complete it, and mail or fax it to us along with a copy of a legal form of identification (passport or driver’s license) with your signature.
