Planning Your Visit

Whether this is your first appointment or you are a returning patient, the best way to get the most out your visit is to come prepared and ask questions.

Your Information

  • Write down all of your health care concerns and questions you’d like to address during your visit.
  • Bring a list of your medications including names, dosages, and how they’re taken.
  • Be prepared to provide your medical history, including dates of any prior hospitalizations or surgeries.

Your Documents

  • Bring a photo ID along with your insurance card.
  • If you’re a new patient and need to transfer medical records from another provider, please download and complete this form

Your Doctor

Questions to Ask your Doctor

Quality health care is a team effort. And as the most important member of your Care Team, you can make sure you get the best possible care by being prepared for your appointment and asking questions before, during and after your visit.

But what questions should you ask? Not to worry, here’s an easy way to get started. First, think about why you are going to see your doctor. Are you going to talk about a health problem? To get or change medications? To get medical tests? To talk about surgery or treatment options?

To help you get the most out of your appointment, we’ve compiled common questions you may want to ask your doctor or Care Team. Simply click the links below, print out the documents and bring them with you when you come in to see us.
