ACPNY is receiving vaccines on a weekly basis from NYC and NYS government agencies. We received a very limited supply for this week and there are no more appointments available for this week’s supply. Please check back next week for updates on future vaccine supply and appointment availability.

COVID-19 vaccines will be approved for distribution to patients over a months-long process. We want to help you understand when, where, and how to seek a vaccination when the time is right. And, we’re offering guidance on how to stay safe and healthy until--and even after--you are vaccinated.  

ACPNY has begun to receive and administer COVID-19 vaccines to specific groups in accordance with the federal and state guidelines. For most patients, appointments for vaccinations will not start for several weeks, depending on eligibility. To see if you are currently eligible to receive the vaccine, please refer to the latest guidance by New York City and New York State.

If you are part of an eligible group, you can schedule an appointment by logging into myACPNY. If you do not have an existing myACPNY patient portal, please register for one. Online scheduling through our patient portal is the fastest and easiest way to schedule.

To prevent bot attacks and fraud, if you are a new patient registering for myACPNY, AdvantageCare Physicians will use a third-party verifier (Experian) to validate your identity. This one-time verification uses identifiers, such as previous employers or addresses, and is not a credit check. If there is not enough information to validate your identity, ACPNY will contact you within three days at the phone number you provide when registering.

Please note that all patients are required to make a COVID-19 vaccine specific appointment. We will not be accommodating walk-ins.

We ask that you do not contact ACPNY about scheduling a vaccination appointment if you are not within a federal and state defined eligibility group. If you are not in an eligible group an appointment cannot be scheduled. Your patience is important because we want to keep our Call Center and appointment scheduling systems open for patients seeking care.

After receipt of the first Moderna dose, ACPNY will schedule your follow-up appointment for the second dose of the vaccine before you leave our office.

Once you are eligible and schedule a COVID-19 vaccine appointment, you will need to:

  • Complete the NYS Eligibility Form in order to get vaccinated. You must show completion of this form when you arrive for your appointment. 
  • Read and complete the COVID-19 vaccine consent form. Bring the completed form with you on the day of your appointment.
  • Review the Moderna Vaccine EUA Fact Sheet for Patients.
  • Bring proof of eligibility to your appointment. This may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status. If you are eligible because of your age, bring a government-issued ID that includes your date of birth (like a Driver’s License or passport) and proof of residence in New York, such as ID, landlord statement, or lease. 

In addition to the steps above, please enroll in v-safeThis isa CDC smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccination. Through v-safe, you can quickly tell the CDC if you have any side effects after getting the COVID-19 vaccine. Depending on your answers, someone from CDC may call to check on you. And v-safe will remind you to get your second COVID-19 vaccine dose at the right time.

Following your COVID-19 vaccine, you will be required to remain in the office for 15-30 minutes. Our nursing team will observe you and assess for any serious adverse reactions.  These are rare.  During the observation period, you will receive your Moderna vaccine card and the team will assist you in scheduling your second vaccine appointment.

The best ways to keep you and your family safe from COVID-19 are to:

  • Wash your hands often and disinfect frequently touched surfaces.
  • Wear a mask over your nose and mouth when around others.
  • Avoid crowds and stay at least 6 feet away from people outside of your household.
  • Monitor your health for any COVID-19 symptoms.
  • Stay home if you are sick.
