Why register for myACPNY

Keep in touch with your care 24/7

As an AdvantageCare Physicians patient, you have access to myACPNY, a 24/7 portal that allows you to:
 

  • Find a provider
  • Schedule appointments
  • Check-in for your appointments at the self-service kiosks in our locations, or even before arriving at our office
  • Review your visit history
  • Request prescription refills
  • View test results
  • Learn from valuable educational materials about conditions related to your health
  • Message your providers and Care Teams, making it easier to stay in touch outside of regular appointments
     

Three easy ways to sign up for myACPNY benefits

Choose one:

You can register with myACPNY on your home computer or any device using a web browser.

  • Go to myacpny.com and click

  • Complete each of the fields with the information requested (paying attention to format guidelines).
  • Answer the five multiple choice questions presented by the Experian Precise ID verification process.* 

    If Experian is unable to verify your identity, you will reach a notification page redirecting you to the log-in page, or offering you instructions for requesting an activation code.

  • Once you’ve correctly answered all questions, you’ll arrive at the sign-up page, where you’ll choose a user name, password, and security question.

* Answering these Experian Precise ID questions will not impact your credit score, and AdvantageCare Physicians will not see your answers. The process is for identification purposes only.

Download and register for the MyChart app from your smartphone or other mobile device.

  • Go to the Apple iTunes or Google Play store and search for the MyChart app from Epic Systems Corp. Click to install, then launch the app.
  • Choose AdvantageCare Physicians from
    the list of nearby healthcare organizations.
    You will only need to make this choice the first  time you use the app.
  • AdvantageCare Physicians will now appear on the My Organizations screen. Click on the logo.
  • At the myACPNY log-in page, choose “Sign Up Now.”
  • On the Sign Up page when asked to “Please Identify Yourself,” skip the fields requesting
    an activation code and click “Sign Up Online” at the bottom of the screen. The Sign Up
    screen will now allow you to type in your name.
  • The “reCaptcha” system will ask you to verify that you are not a robot.
  • Answer the five multiple choice questions presented by the Experian Precise ID verification process.*
  • Click “Next,” then enter your username, password, and security questions.
  • Click “Next” and accept the user terms and conditions.
  • You’re finished and ready to use your account.

* Answering these Experian Precise ID questions will not impact your credit score, and AdvantageCare Physicians will not see your answers. The process is for identification purposes only.

When you show up for your first appointment, or even before that, sign up for myACPNY at one of our self-service kiosks at any AdvantageCare Physicians location.

  • On the home page, click

  • Choose a user name and type it in.
  • Choose and enter a password.

  • Select and answer a security question for future identification.

  • Provide your email address.

  • Thank you – you’ve completed the registration process.
